Your Health Journey

What It's Actually Like to Work with a Longevity Physician

Five clinical phases. One real relationship. A different way to think about the next twenty years of your health.

Most medicine happens in fragments — a 15-minute visit, a stack of labs, a referral that takes three months. This is the opposite of that. Here's exactly what working with me looks like, from your first conversation forward.

The Problem Isn't You. It's the System You've Been Using.

If you've been frustrated with your health, it's probably not because you aren't trying. It's because the way modern medicine is delivered makes it almost impossible to get a coherent picture of what's actually happening inside your body.

The 23-Second Interruption

The average primary-care visit is 15–20 minutes — but research shows doctors interrupt patients after just 23 seconds. Only one in four patients ever gets to finish describing what brought them in.

"Your Labs Are Normal"

"Normal" is a population average that includes everyone who's getting sicker. It's the floor of disease, not the ceiling of health. You deserve to know your real numbers.

Supplement Roulette

You buy bottles based on podcasts and Instagram. Some help. Most don't. Without data, you're guessing — and your liver is doing the testing.

The Specialist Maze

Three doctors, three plans, none of them talking to each other. No one is running the long game on your behalf. You become the project manager of your own health by default.

The cost of doing nothing isn't a sudden crisis. It's a slow drift — five years from now, the same labs, the same fatigue, the same uncertainty, and a body that's quietly aging faster than it should.

This isn't a 15-minute appointment.
It's a relationship.

Working with me is different by design. We move through five distinct phases together, each with a clear purpose. You won't be guessing what's next. You'll know exactly where you are, what we're doing, and why.

The pace flexes to fit your life. The structure does not. You don't have to commit to all five phases upfront — you commit to the first conversation, and we go from there.

What you get isn't a protocol. It's a strategy — built from your data, for your body, in partnership with a board-certified physician who's spent two decades on this work.

The Five Phases

From first conversation to sustained vitality

Each phase has a purpose. You move forward when you're ready — not on an arbitrary timeline.

01
Discovery

We figure out what actually matters to you

We start with a one-on-one Initial Consultation. No assumptions, no script. I ask about your history, what you're noticing in your body, what's worked, what hasn't, and what you actually want out of the next five, ten, twenty years. I review whatever data you already have — old labs, family history, wearable trends, supplements you're taking. By the end of this phase, we've named the real priorities, and you know what we'll need to assess next.

  • 30-minute Initial Consultation
  • Goals & priorities mapped
  • Existing data reviewed
  • Next-step recommendations
You walk away with Clarity on what to investigate first — and whether we're a fit to keep working together.
02
Assessment

We measure what we need to measure

You can't optimize what you haven't assessed. In this phase we gather the data that will drive every decision afterward — a comprehensive lab panel through ULTA Lab Tests, optional epigenetic testing through TruDiagnostic (your TruAge™ biological age), genetic insight through FitnessGenes when relevant, and a structured look at any wearable data you bring (Oura, WHOOP, Apple Watch). Then we sit down for a Physician Interpretation Session and walk through every number, in plain language, together.

  • Tailored lab panel ordered
  • Epigenetic & genetic testing (optional)
  • Wearable data integration
  • Physician Interpretation Session
Here's what this looks like in practice: a few days after your labs come back, you'll get an email from me with a written summary attached — biomarker by biomarker, what each number means for your specific body, and the three things I want to discuss when we meet. You'll read it on your phone before work, and probably feel — for the first time — like someone actually looked at you.
You walk away with A complete map of your current biology — and the first real understanding of what's driving how you feel.
03
Strategy

We build the plan that's yours

With the data in front of us, we design a tailored plan across all four biological layers — nutrition, movement, sleep, stress, supplementation, and where appropriate, lab-targeted interventions. This isn't a generic protocol pulled off a shelf. It's built from your specific genetics, your specific markers, your specific life. You leave this phase with a written strategy you can actually follow, and you know what success will look like at our next check-in.

  • Tailored written plan
  • Targeted supplement protocol
  • Lifestyle blueprint by layer
  • Onboarding into ongoing consulting
You walk away with A written plan you can hold in your hand — and a clear sense of what to start, stop, and measure.
04
Optimization

We iterate, retest, and refine

Real change shows up in your biology over weeks and months, not days. In the Optimization phase we meet regularly — monthly for most clients — to look at how you're responding, what your wearable trends are telling us, and what life has thrown at you since we last spoke. We retest the markers that matter. We adjust what isn't working. We double down on what is. This is where the four-layer model stops being a framework and starts being how you live.

  • Monthly check-in sessions
  • Strategic retesting
  • Plan adjustments based on data
  • Wearable trend reviews
You walk away with Trend lines moving in the right direction — and confidence that what you're doing is actually working.
05
Sustained Vitality

You stop chasing health. You live it.

The destination isn't a perfect lab panel — it's a way of living where vitality is the baseline, not the goal. By this phase, you've internalized the four-layer framework. You read your own data confidently. You know what your body needs and when. You're not anxious about every new study or trend. We meet less often, but with more depth. I become a trusted advisor for the long arc of your health — the partner you call when something changes, before it becomes a problem.

  • Long-term advisory partnership
  • Annual deep-dive reviews
  • Sustained biological-age improvement
  • You as the steward of your own data
You walk away with A life where health isn't a project anymore. It's how you live.

The First Step

What's included in your Initial Consultation

A 30-minute conversation with a board-certified physician. Here's exactly what you get for $150.

Your $150 Discovery Call Includes

  • 30 minutes of one-on-one time with Dr. Kilgore — not a nurse, not an intake coordinator, not a chatbot.
  • A structured history review covering your current health, goals, prior labs, family history, and what's brought you to this conversation.
  • Review of any existing data you bring — old labs, wearable trends, genetic results, supplement lists.
  • Three concrete priorities identified before we hang up — what matters most, in order, and why.
  • A written summary email after the call so you don't have to take notes.
  • Clear recommendations for what to do next — whether that's working with me further or simply taking the priorities to your own physician.

Bonus Included

The Quantitative Health Lab Ordering Guide (PDF) — sent to you before our call. So when we talk, you'll already have a starting framework for the kinds of testing that match your goals.

The "Three Priorities" Promise

If we don't identify at least three concrete priorities for your health in our 30 minutes together, the consultation is on me. Full refund, no questions asked.

Honest Self-Qualification

Is this the right path for you?

Working with me isn't right for everyone. Read both columns before you book.

This is for you if…

  • You want a long-term partnership, not a one-time fix.
  • You're willing to get bloodwork and look at your own data.
  • You believe biology is measurable and worth understanding.
  • You've felt unheard or rushed in conventional appointments.
  • You'd rather invest in prevention now than treatment later.
  • You're ready to be a participant, not a passive patient.

This isn't for you if…

  • You want a quick fix or a magic supplement.
  • You're looking for primary care or an acute-care doctor.
  • You're unwilling to do bloodwork or follow a written plan.
  • You expect to outsource the work without doing your part.
  • You aren't comfortable making data-informed decisions.
  • You aren't ready to think in years, not weeks.
What This Asks of You

The journey works if you show up for it

I bring the framework, the data interpretation, and the clinical judgment. You bring four things.

Honesty

About what you're eating, drinking, sleeping, and skipping. The plan only works on the truth, and the truth is never as bad as you think.

Patience

Your biology took years to get where it is. Meaningful change is measured in months, not days. We move fast where we can and slow where it matters.

Participation

You'll get bloodwork done. You'll complete the intake. You'll show up to the sessions. I'm the guide — the work is yours.

Perspective

The willingness to think in years, not weeks. This work is about the next decade of your health, not the next month. Long-game thinking is the lens that makes the rest of it work.

Common Questions

Before we start

How long does the full journey take?

Discovery and Assessment typically span the first 4–8 weeks. Strategy is built in the weeks that follow. Optimization is ongoing — most clients work with me on a monthly cadence for at least 12 months before transitioning to Sustained Vitality. There is no rush. The right pace is the one that fits your life.

Do I have to commit to every phase upfront?

No. You begin with the Initial Consultation. After that conversation, you decide whether to continue. Many people move forward, some don't, and both are fine. This is a partnership you choose into deliberately.

Is this a replacement for my primary care doctor?

No. I work alongside your existing care team. I'm a board-certified internist focused on longevity, biological aging, and the integration of advanced testing into a coherent strategy. For acute issues, emergencies, and routine primary care, your local physician remains essential.

Where can you practice?

I currently hold active medical licenses in Michigan, Alabama, Missouri, Wisconsin, and Pennsylvania. Services are educational and consultative in nature outside of those states; clinical care is provided where licensure permits.

What does it cost?

The Initial Consultation is $150. The Physician Interpretation Session for lab review is $225. Ongoing consulting tiers range from $500 to $2,000 per month depending on cadence and depth. Full details are on the consulting page.

Do you take insurance?

No. Working outside of insurance means I can spend the time with you that this work actually requires — and you're never reduced to a billing code. Many clients use HSA/FSA funds for consultation costs.

What if I'm not sure where I'd fit?

That's exactly what the Initial Consultation is for. We'll know together by the end of that conversation — and if it's not a fit, you'll leave with three clear priorities anyway. That's the promise.

Start with one conversation.

The Initial Consultation is the first phase — and the only commitment you need to make today. If it's not a fit, you've still left with three priorities and a refund. Either way, you move forward with clarity.

P.S. — A note from Dr. Kilgore

One last thing. I take new clients deliberately because the relationship is what makes this work — for both of us. If you've read this far, you're already the kind of person I want to be working with: someone who takes their own health seriously and wants more than a rushed appointment where you're interrupted before you finish your first sentence.

The Initial Consultation is 30 minutes of real time, one-on-one. I'll show up prepared. I hope you will too. And whatever you decide afterward, you'll leave the call further along than you were when you started.

I'm looking forward to the conversation.
