Depression & Anxiety Assessment
A confidential, physician-designed mental health screening using two of the most validated instruments in clinical medicine. Takes about 3 minutes.
⚠️ Before You Begin — Please Read
MindScreen™ is a screening tool, not a diagnosis. The PHQ-9 and GAD-7 are validated instruments used by physicians worldwide to identify symptoms of depression and anxiety, but a screening result does not replace a clinical evaluation by a licensed mental health professional or physician.
If your symptoms are significantly impacting your daily life, relationships, or work, please reach out to a qualified provider — even if your screening score appears mild.
What This Screening Measures
Depression and anxiety are the two most common mental health conditions in the United States, affecting more than 40 million adults each year. Both are highly treatable — but only when identified. Combined PHQ-9 and GAD-7 screening is the gold standard in primary care for catching symptoms early, before they progress.
PHQ-9 — Patient Health Questionnaire
A 9-item depression screener developed by Drs. Spitzer, Kroenke, and Williams. Used in over 4,000 published studies and recommended by the U.S. Preventive Services Task Force. Scores range from 0–27 and identify five severity bands from minimal to severe depression.
GAD-7 — Generalized Anxiety Disorder Scale
A 7-item anxiety screener also developed by the same team. Validated against structured clinical interviews and used by physicians, psychologists, and researchers worldwide. Scores range from 0–21 and identify four severity bands from minimal to severe anxiety.
Not Sure If You Should Screen?
If you've been feeling down, worried, exhausted, or "not yourself" for two weeks or longer, screening is appropriate. There is no minimum severity required — early identification leads to better outcomes. If you'd prefer to discuss your situation with a physician first, Dr. Kilgore offers confidential consultations.
Note: ULTA Lab Tests not currently available in NY, NJ, RI, or HI.
